Complaint form about discrimination and/or disadvantage
If you have witnessed or have been a target of innapropriate behaviour, we are very sorry that happened and we want to hear from you.
The form below is anonymous, the inclusion of personal data is completely up to you. You should only add your email and name, if you want us to get in contact with you about the occurence.
We ask you to inform us though what persons have conducted themselves innapropriately and the general time of the occurence. We also ask you to describe the occurence to the extent that you deem necessary and are comfortable with, though the more information we have, the best prepared we are to respond to it.
